Nomination Form for 2007 Humanitarian Award

The criteria for nominating a person for the Humanitarian Award are as follows:

1. The winner shall be a member of a faith group.

2. The winner shall not be a current member of GSIA.

3. The winner shall be chosen for his/her service to those in human need in the community.

4. It shall be irrelevant to the judgment of the committee whether the winner has been compensated for his/her service or is a volunteer. In the event the winner is compensated for his/her services, the award shall go to his/her organization or agency.

5. The award may be given to organizations, agencies, and/or individuals.

6. The winner shall not have received the Humanitarian Service Award in the previous seven (7) years.

7. Submission deadine is Wednesday, August 22nd.
